Yes. Sumac Online Forms put your intake, referral and consent forms on your website. Each submission creates or updates the client record in Sumac, so nobody types it in again. One intake form is included, and more forms are an add-on.
Sumac runs on Amazon Web Services in Canada. Data is encrypted in transit and at rest, permissions by role and by program control who sees what, an audit trail records every view, edit and delete, and backups run every night and are kept for five years.
